Audio-Visual Equipment Request Form
for The Delaware Center Only

All audio-visual equipment must be reserved at least one week in advance. All classrooms are equipped with overhead projectors. Requests must be submitted to Donald Bahizi by completing the on-line Web form.  NOTE: All fields must be filled in before you submit the form, or you will be required to go back and fill in any missing fields.  For questions, please contact Donald Bahizi at 614-247-9006 or bahizi.3@osu.edu. Please fill a separate request for each course.
